To order 2008-09 SPPS volunteer recognition certificates, please fill out the form below. Certificates are still available.

If needed, the certificate is posted below in PDF so schools/programs can print them on your own.  Please note: This is a large file--for best results, download the file to your computer and then open it.

Number of certificates needed: Your response to this question must be in numbers only. If it contains any other characters or spaces, this form will not work. (Please note: Certificates will be distributed in bundles of 25. If your order is not divisible by 25, it will be rounded up.)
